Japan’s Stem Cell Awakening

In a quiet operating room on Kyoto University’s medical campus, a team of researchers slipped a syringe of lab-grown neurons into the brain of a living person.

Leading the trial was Takahashi Jun, a neurosurgeon with the kind of demeanor you’d expect from someone whose job involves rewiring minds. His team had spent five years coaxing adult skin cells back into an embryonic state, then training them to become the dopamine-producing neurons that Parkinson’s disease steadily erodes. Now, those donor cells were inside seven patients. And they were working.

At a press conference in April, Takahashi was characteristically reserved. “Confirming therapeutic efficacy is an important step,” he told reporters, “but not the final goal.” He meant it clinically. But the sentence hung in the air like a koan.

In pristine labs across Japan, scientists are tinkering with the architecture of regeneration. They’re growing retina strips in dishes, sculpting sheets of cardiac tissue, rehearsing the choreography of cellular repair.

And if the tone feels hushed, the ambition is anything but. The country wants to rebuild its national vitality from the molecular level up. Is Japan aging? Yes. Shrinking? Certainly. But decaying? Not if stem cells have anything to say about it.

The spark came in 2006, when Yamanaka Shinya, Takahashi’s colleague at Kyoto University, reprogrammed ordinary skin cells into induced pluripotent stem cells (iPSCs) – capable of morphing into virtually any tissue in the body. They didn’t require embryos. And, more curiously, they seemed to ignore the limits of age and immune mismatch.

Japan had stumbled upon a way to sidestep the biological and bioethical barriers that dogged stem cell research elsewhere. Almost overnight a nation often portrayed as graying and cautious found itself steering something radical.

The government had already begun backing Yamanaka’s work quietly. After he won the Nobel Prize in 2012, it only doubled down. Subsidies and grants poured into university labs and startup ventures. The work was no longer just personal or even medical. It had become bigger: a story Japan could tell itself about its place in the world.

Prime Minister Abe Shinzo gave that future a name. In 2013, he pledged to create a “society of good health and longevity” – and to export it globally. Regenerative medicine, he declared, would be the “key.”

Policy followed suit. In April 2014, Abe’s government cleared a path for iPSC treatments to reach patients after preliminary trials. A country known for caution was, suddenly, testing the limits of its own restraint.

Still, no therapy had yet touched a human body.

Takahashi Masayo moved first, leaning into the moment with quiet conviction. She had been there from the beginning, helping drive Yamanaka’s iPSC discovery in Kyoto alongside Takahashi Jun – her partner in both life and science. But after the celebrations, she set out for Kobe to build something new: Vision Care, a research haven for sight.

Stem cell transplantation: Why India needs more donors

Every year in India, thousands of children are born with blood disorders like thalassemia major and sickle cell anemia—conditions that condemn them to a life of regular blood transfusions, chronic pain, social stigma, and premature death unless they receive a curative treatment: a stem cell transplant.

While medical science today offers hope, access to that cure remains elusive for most Indian children, not because the treatment doesn’t exist, but because some don’t have donors and others do not have enough funds to afford the treatment.

The first issue—the lack of donors—can be addressed very effectively if people come forward. Unrelated stem cell donors can help thousands of patients who cannot find matched donors within the family.

India has the highest number of thalassemia major patients in the world, with an estimated 100,000 requiring lifelong blood transfusions and chelation therapy. Each year, 10,000 to 12,000 children are born with the condition. Sickle cell anemia—another severe and painful genetic blood disorder—primarily affects tribal and rural populations across states such as Chhattisgarh, Odisha, Maharashtra, Madhya Pradesh, Gujarat, and Telangana.

These are not rare diseases, yet often overlooked public health challenges. While gene therapy and new drugs offer hope for the future, the most definitive cure currently available for these children is a stem cell transplant, also known as a bone marrow transplant.

A common barrier to donor registration in India is fear—of pain, of long-term health effects, and of the unknown. But, the fact is donating stem cells today is no longer the invasive, painful process it once was.

In over 90% of cases, we now use Peripheral Blood Stem Cell Collection (PBSC)—a process much like donating blood or platelets. Before donation, the donor is given a medication called G-CSF (Granulocyte-Colony Stimulating Factor) for five days. This helps increase the number of stem cells in the bloodstream.

People often express concern about G-CSF, fearing long-term side effects. But G-CSF has been used safely in healthy donors for decades. The side effects are generally mild and temporary—such as body aches, fatigue, or mild fever—and resolve quickly after donation. Global data from hundreds of thousands of donors show no significant long-term risks. Stem cells regenerate naturally within a few weeks.

Despite the safety and simplicity, myths prevail in India—fuelled mostly by a lack of awareness and cultural resistance to donation. Many families refuse to let even willing young adults donate, fearing future infertility or weakness, none of which are backed by science.

The success of a stem cell transplant depends heavily on finding a genetically matched donor. Unlike blood donation, a match isn’t determined by blood group but by HLA (Human Leukocyte Antigen) typing. The odds of finding a match improve significantly when the donor is from the same ethnic background. This is why India needs Indian donors—matches are ethnically specific.

Exploring Placenta-Derived Stem Cell Therapy

A phase I clinical trial has investigated the safety and feasibility of intravenous placenta-derived mesenchymal stem cells (PLMSCs) in patients with secondary progressive multiple sclerosis (SPMS). The study, conducted at Tehran University of Medical Sciences, involved five individuals with SPMS who had not responded to conventional disease-modifying treatments.

PLMSCs were selected for their immunomodulatory and anti-inflammatory properties, as well as their potential neuroprotective effects. The cells were isolated from full-term placentas following cesarean deliveries and expanded under GMP conditions. Each patient received a single intravenous dose of 3 million PLMSCs per kilogram of body weight.

The primary endpoint of the study was safety and tolerability over a six-month follow-up. No serious adverse events were reported.

Secondary, exploratory outcomes included assessments of neurological function, cognitive and psychological performance, imaging changes, and immunological markers. Neurological disability was measured using the Expanded Disability Status Scale, which showed a statistically significant decrease over six months (p < 0.0001). Cognitive improvements were observed in tasks related to memory, verbal fluency, and executive function. Fatigue and psychological symptoms such as hostility and somatization also showed reductions.

Neuroimaging analyses provided additional insights. Functional MRI revealed increased connectivity in several brain regions, particularly in the default mode and parietal networks, areas associated with memory and spatial processing. Diffusion tensor imaging (DTI) detected a significant decrease in radial diffusivity in the normal-appearing white matter of the left hemisphere (p = 0.0186), a potential indicator of improved axonal integrity. However, no significant changes were noted in total brain volume or lesion load.

The authors emphasize that the trial was designed to assess safety, not efficacy. While the preliminary findings suggest possible clinical and immunological benefits, the small sample size and lack of a control group limit generalizability. Further randomized, controlled studies are needed to evaluate therapeutic efficacy and understand underlying mechanisms.

Discovery offers new insights into skin healing and regeneration in Atlantic salmon

University of Stirling scientists have discovered cells in the skin of Atlantic salmon that offer new insights into how wounds heal, tissues regenerate, and cellular transitions support long-term skin health. By understanding how skin cells remodel and heal tissue, researchers hope to develop new strategies to enhance tissue integrity and reduce non-healing wounds, potentially improving salmon farming mortality rates.

Research led by Dr Rose Ruiz Daniels of the University’s Institute of Aquaculture uncovered the previously unknown population of stem cells. The study reveals that fibroblast-like stem cells – known as mesenchymal stromal cells (MSCs) – play a central role in the remodelling phase of wound healing, which is critical for restoring skin integrity following injury.

Using advanced cell profiling technologies – single-nucleus RNA sequencing and spatial transcriptomics – the researchers profiled skin cells during a wound healing time course.

“We found MSCs at both the wound site and in intact skin, suggesting these adult stem cells are a stable and functional part of salmon skin, and likely to be involved in maintaining its barrier and structural properties,” said Dr Rose Ruiz Daniels. “These cells become more transcriptionally active during the remodelling stage of healing and show signs of differentiating into multiple tissue types, including bone and fat. This hints at a broader regenerative capacity in fish skin than previously understood, potentially linking repair processes in the skin to those in deeper tissues like muscle, scales, and connective tissue.”

MSC-like cells are well characterized in mammals, but this study suggests that in teleost fish such as salmon, these cells may retain higher pluripotency – or stemness – meaning they can take on a wider range of regenerative roles. This raises the possibility that fish skin regeneration may operate through more flexible cellular pathways than in terrestrial vertebrates.

The study also maps the spatial niches of various MSC subclusters, laying the groundwork for future studies aimed at manipulating these cells to improve tissue repair, resilience, and overall fish health.

“These findings have potentially far-reaching implications for aquaculture. Barrier tissue health, particularly of the skin and gills, is a major challenge in Atlantic salmon farming and a leading cause of mortality in sea cages,” said Ruiz Daniels. “There is an urgent need for innovative biotechnological approaches to enhance fish health, as aquaculture also faces mounting challenges from climate change, including heightened disease risks and increased thermal uncertainties. By understanding how skin cells remodel and heal tissue, we hope to develop new strategies to enhance tissue integrity and reduce non-healing wounds at barrier tissues.”

This study is a collaborative effort across institutions, including the University of Stirling, the Roslin Institute, Nofima, and the University of Prince Edward Island.

Blood Aging Revealed Through a Novel Epigenetic Clonal Tracing Method

The blood system changes over time: a subset of stem cells outcompete their neighbors and gradually take over blood production. In addition, the blood stem cell reservoir shrinks and becomes dominated by clones that show a preference for producing myeloid cells, immune cells linked to chronic inflammation.

“Our blood stem cells compete for survival,” noted Lars Velten, PhD, group leader at the Centre for Genomic Regulation (CRG) in Barcelona. “In youth, this competition produces a rich, diverse ecosystem, while in old age, some drop out entirely. A few stem cells take over, and these work extra hard to compensate. This reduces diversity, which is bad for the blood system’s resilience. Diverse stem cells can respond to different stresses, so the dominance of a handful of clones makes the whole system more fragile.”

In youth, humans have between 50,000 to 200,000 active blood stem cells, which create between 100 to 200 billion new blood cells every day. Tracking every blood cell back to its original stem cell requires genetically modifying DNA.

The researchers found that “DNA methylation of a subset of CpG sites reflects cellular differentiation, whereas another subset undergoes stochastic epimutations and can serve as digital barcodes of clonal identity.” They write that targeted single-cell profiling of DNA methylation at single-CpG resolution can accurately extract both layers of information.

In turn, the team developed a new technique, EPI-Clone, which reads methylation barcodes from individual cells. It was built by modifying Mission Bio’s single-cell sequencing platform Tapestri. They used it to reconstruct the history of blood production in both mice and humans, helping trace which stem cells contributed to making blood. More specifically, they captured “hundreds of clonal differentiation trajectories across tens of individuals and 230,358 single cells.”

In young blood, thousands of different stem cells contribute to a rich and diverse pool of red blood cells, white blood cells, and platelets. But EPI-Clone revealed that in older mice, up to 70% of blood stem cells belonged to just a few dozen large clones, compared with around 50% in younger mice.

In humans, the exact percentage varied between the dozen healthy donors between 35 and 70 years old. The study found that by age 50, many blood stem cells begin to drop out and larger clones begin to take over, while by age 60 and beyond, the shift becomes even more pronounced. The authors suspect the loss of clone diversity could help explain “inflammaging,” the persistent chronic inflammation that emerges during aging and which can make people more vulnerable to disease.

Targeting the clock: new drug disrupts glioblastoma stem cells

Glioblastoma, the most aggressive form of brain cancer in adults, is notoriously difficult to treat. While most patients undergo surgery, chemotherapy, and radiation, tumours often return, driven by a stubborn population of glioblastoma stem cells.

In a series of preclinical studies published in Neuro-Oncology, an international research team led by Dr Steve A. Kay of the Keck School of Medicine of USC and Dr Jeremy Rich of the University of North Carolina at Chapel Hill announced that a new compound, SHP1705, disrupts the circadian clock machinery glioblastoma cells rely on to survive and multiply.

Clock proteins: a new vulnerability in cancer
Circadian clock proteins help regulate the body’s internal rhythms, including sleep-wake cycles and cellular functions. But in glioblastoma, cancer stem cells hijack these proteins, particularly CRY2, to drive tumour growth and resist treatment. SHP1705, a CRY activator, is designed to restore activity to this suppressed protein, essentially turning the cancer’s own tools against it.

“We have mounting evidence that clock proteins can be co-opted by brain cancer stem cells to fuel their growth,” said Dr Kay. “If we can successfully target the circadian clock, these cells lose their ability to replicate.”

Unlike earlier clock-targeting drugs, SHP1705 selectively boosts CRY2 activity in glioblastoma cells without disrupting healthy brain cells, where CRY2 is already active. This precision targeting is key to the drug’s effectiveness, and its safety profile.

Promising preclinical results
In lab tests, SHP1705 impaired the survival of glioblastoma stem cells while sparing normal cells. It worked on both chemotherapy-sensitive and chemotherapy-resistant glioblastoma cell lines, showing potential as a treatment, even when tumours have relapsed after standard care.

Animal studies supported these findings, with mice treated with SHP1705 exhibiting slower tumour growth and longer survival. The compound also appeared to increase the cancer’s sensitivity to radiation therapy, making existing treatments more effective.

Researchers also tested SHP1705 in combination with another circadian-targeting drug, SR29065, developed at the Herbert Wertheim UF Scripps Institute. Together, the two compounds produced even better results, suggesting a powerful synergy that could be explored in future therapies.

A step closer to the confident production of blood stem cells for regenerative medicine

Stem cells can produce any other cell type, it is just a matter of telling them in the right way. From a biological perspective, this means activating the proper genetic program by pressing the right keys, that is, the right genes, at the right moment. Quite often, blood cancer patients require the replacement of their blood stem cells in the bone marrow, the tissue producing blood cells where their cancer grows.

Unfortunately, finding a compatible donor happens to be too challenging sometimes. What if we could produce the cells that make blood in the lab, right from basic stem cells, and use them to regenerate new and healthy bone marrow?

To do this, you would need to know what genes to activate in a stem cell. In a tour de force, the team led by Dr. Anna Bigas screened thousands of genes in the mice genome to see which were able to transform an embryonic stem cell into a blood precursor or, more technically, a hematopoietic stem cell (HPSC). The screening identified a group of seven genes apparently able to accomplish the task.

In subsequent experiments, the team confirmed that the timely activation of the seven genes was sufficient to transform mouse embryonic stem cells into HSPC, and that these newly produced cells were able to regenerate and sustain a functional blood system, producing all kinds of blood cells, including the immune lineages, in adult mice.

The research, published in the journal Blood, was first-authored by Dr. Luis Galan Palma, a researcher from the Bigas Lab, in collaboration with other researchers including Dr. Clara Bueno and Dr. Pablo Menendez, experts in developmental and pediatric leukemia, also at the Josep Carreras Institute.

Dr. Bigas is confident that the results obtained in mice can be translated into the human system since, despite the differences, the mechanisms driving stem cells differentiation are so fundamental that they are shared between species.

Dr. Bigas says, "We know that those genes are also present in the human genome, and they are highly conserved. That is, their genetic sequences are almost identical." Research is on the way, but chances are they really play the same role in humans as in mice.

This research is a proof-of-concept for the Bigas Lab's project "Making Blood," a highly ambitious endeavor to develop a technological platform aimed at the production of human HSPC off the shelf. If successful, we are just a few years away from a new era in the treatment of leukemia and other blood disorders, based on the long-awaited regenerative medicine.

Safety of Stem Cell Therapy for Parkinson’s

Although further clinical trials will be needed to demonstrate the efficacy of these therapies, brain transplantation of dopamine-releasing neurons is a promising treatment.

Almost indefinite proliferation, as well as the ability to give rise to any cell type, are characteristics of pluripotent stem cells. Embryonic cells naturally possess this characteristic, as they are destined to form a complete organism. However, the induced pluripotent stem cells (iPSCs) discovered by S. Yamanaka, for which he was awarded the Nobel Prize, have these same characteristics.

The journal Nature reports on the clinical trials by Sawamoto et al. 1 and Tabar et al. 2, which, using pluripotent stem cells, seek to treat Parkinson’s disease, a neurodegenerative disease that affects 8.5 million people worldwide.

More than 100 clinical trials are already underway to obtain products derived from pluripotent stem cells, most of which focus on the eye, the central nervous system, and cancer. Potential therapies for Parkinson’s disease are the most advanced among neurodegenerative diseases of the central nervous system. Parkinson’s disease is characterized by the increasing decrease in dopaminergic neurons. The dopaminergic system is made up of a set of neurons that produce, release, and respond to dopamine. This system has several pathways; one of them is the nigrostriatal pathway, which connects the substantia nigra to the striatum. This pathway is crucial for movement control, and its malfunction is associated with Parkinson’s disease. Slowness of movement, muscle rigidity, gait disturbances, and tremors are symptoms associated with this condition.

As is known, levodopa, a precursor of dopamine that circulates from the bloodstream to the brain, is usually administered to treat Parkinson’s disease. The effectiveness of this treatment is decreasing due to the scarcity of dopaminergic neurons. For this reason, transplantation of dopamine-producing cells could be a valid therapy. As early as the 1980s, cells from the fetal ventral midbrain, where dopaminergic neurons are abundant, were transferred to the striatum of Parkinson’s patients. The result was increased dopamine levels and improved motor function for many years.

In addition to the technical challenges of obtaining the quantity and quality of cells for transplantation, the use of fetal brain tissue also raised ethical concerns. One possible solution is the generation of dopaminergic cells using pluripotent stem cells, capable of multiplying abundantly. Currently, many research groups have easily produced dopaminergic neurons from human pluripotent stem cells and transferred them to animal models of Parkinson’s, thereby achieving recovery of their functions. In 2020, an individual received dopaminergic neuron precursor cells obtained from cells in their epidermis, derived from induced pluripotent stem cells. Subsequently, clinical trials have been conducted to verify this initial experiment.

In the first Nature article we referenced, Sawamoto et al. report on a Phase I/II trial conducted in Japan. They worked with induced pluripotent stem cells to obtain dopaminergic progenitor cells and transplanted them into both cerebral hemispheres of seven people with Parkinson’s disease. Although the primary objective was safety and potential adverse effects, they also reviewed dopamine levels and motor symptoms. The transferred cells produced dopamine and did not cause tumors (the possibility of tumor formation is always a risk associated with pluripotent stem cells). No serious adverse effects were reported. On the other hand, a decrease in motor symptoms was observed. Of the six participants, five showed improvement in their motor skills, which continued when they discontinued their standard medication.

In the other article we referred to, Tabar et al. reported the results of a Phase I clinical trial. Twelve participants from Canada and the US received dopaminergic precursor cells from an embryonic stem cell line. A 50% reduction in symptoms was observed at 18 months in those who received a high dose. The survival of the transferred cells and the increase in dopamine production were notable. It is also important to note that no involuntary movements (dyskinesia) were observed, as is often the case with this type of transplant.

These two studies show that allogeneic transplants (from donors) for treating Parkinson’s appear to be safe. Of course, their efficacy still needs to be confirmed by further research. It must be taken into consideration that these studies were conducted in small, open-label populations (researchers and participants were aware of the treatment they were receiving), so the placebo effect or researcher bias could have influenced their results. However, one undeniable fact is that both clinical trials demonstrated safety and indicated potential efficacy. A more complete assessment will have to wait until the completion of phases II and III, but this does not prevent us from pointing out that we are witnessing an important step toward establishing this type of cell therapy for Parkinson’s.

From a bioethical perspective, however, we must emphasize that work with induced pluripotent stem cells does not deserve the same evaluation as work with human embryonic stem cells. As we know, obtaining embryonic stem cells from the inner mass entails the death of the embryo, the tiny human organism that, if unimpeded, will develop normally until birth. Therefore, it is not lawful to carry out these types of actions that truncate the life of a human being in its infancy.

Newly discovered ‘super stem cells’ can transform into better versions of themselves

Scientists have discovered a simple yet powerful way to make stem cells stronger, healthier, and more capable than ever—by changing their diet. The result is a new class of super stem cells that outperform their standard counterparts in nearly every way.

Researchers from the University of Copenhagen have developed a method to enhance embryonic stem cells by altering the sugar in their growth medium. Instead of glucose, the team fed the cells galactose, forcing them to rely on a different energy pathway called oxidative phosphorylation. This small metabolic shift had a major impact, reprogramming the cells to behave like younger, more adaptable versions of themselves.

These super stem cells are better at transforming into specialized cell types like liver, nerve, or skin cells. They also remain healthier over time in culture, resisting the gradual decline that typically limits the usefulness of standard stem cells. According to the researchers, the metabolic change effectively resets the cells to an earlier developmental stage, boosting their performance and versatility.

What’s happening inside these cells is just as remarkable. The diet change activates proteins associated with aging and improves how genetic information is processed. Stem cell transplants have emerged as a vital treatment option, and often the only cure, for many patients battling blood cancers such as leukemia, lymphoma, and myeloma.

Stem cells are often described as the body's master cells. These cells have the unique ability to develop into various types of cells, including red blood cells, white blood cells, and platelets, all of which are essential for survival.

"Stem cells are the parent cells of all blood cells in the body," explained Dr. Gopinath, Hemato-Oncologist at MGM Cancer Institute, Chennai. "They produce red blood cells that carry oxygen, white blood cells that fight infections, and platelets that help in clotting. They also support tissue healing and fight inflammation," he added.

HOW STEM CELLS HELP TREAT BLOOD CANCER

Treatments like chemotherapy and radiation aim to kill cancer cells in the marrow but often end up damaging healthy cells too, wiping out the patient’s ability to produce new blood.

"In such cases, we match a donor and infuse their healthy stem cells into the patient. These donor cells not only regenerate the patient’s blood system but also help destroy any remaining cancer cells, a phenomenon known as the ‘graft-versus-leukemia’ effect," said Dr. Gopinath.

According to experts, stem cell donation is often the only curative option for patients with aggressive or relapsed blood cancers. Patients who cannot use their own cells, particularly when their marrow is cancerous, rely entirely on donors.

That’s where matching stem cell registries and volunteers step in.

“Stem cell donation helps rebuild a healthy immune system after intense cancer treatments. It allows doctors to use high-dose chemo or radiation safely, knowing that healthy stem cells can restore the blood system afterward,” said Dr. Deepak Kumar, Associate Professor at School of Pharmaceutical Sciences, Shoolini University.

Dr Kumar added that bone marrow transplants, a form of stem cell therapy, have long been used to treat blood-related diseases.

"In addition to blood cancers, stem cells are also being explored in areas like spinal cord repair, osteoarthritis, and even diabetes research. Their regenerative potential is immense," he said.

Two years ago, 25-year-old Abdul Khader from Calicut paused his Ramadan fast to donate stem cells, a decision that saved a life and brought new hope to a family.

Abdul had registered as a potential donor with DKMS Foundation India in 2018. In 2022, he was found to be a match for a 50-year-old man in Kochi suffering from myelofibrosis, a rare blood cancer where scar tissue builds up in the bone marrow, making it difficult to produce healthy blood cells.
